SURGERY FEES:
Payment for surgery is to be paid in full before the surgery is performed. To secure your desired date, a 20% deposit is required; it will be applied toward your payment balance. Payment for the balance of the surgery fee is due 3 weeks prior to the procedure date.
PAYMENT OPTIONS:
Zeeba Clinic accepts VISA, Mastercard, American Express, Discover, money orders, cashiers and personal checks, and CareCredit payments.
INSURANCE:
Dr. Guyuron is not a contracted provider with insurance companies. As a courtesy, we will send you your medical claim form, with your supporting documentation, so that you may submit your claim to insurance for possible out-of-network benefits reimbursement. You should receive this documentation within three weeks of your surgery.
